The record we can pull on our own is limited and often misses out-of-state or older history. Until your driving history is verified, we can only start you at entry-level mileage rates, no matter how long you have been driving. Your driving record, accidents, violations, and employment history are checked against your MVR from every state you have been licensed in, your PSP report, the FMCSA Drug and Alcohol Clearinghouse, and your previous DOT-regulated employers. What you tell us here is compared line by line against what those records say.

If what you report does not match what the records show:

→Your application is denied. If you have already started, your employment ends. Federal law requires this application to be truthful under 49 CFR 391.21, and falsifying it disqualifies you.
→Your pay is set from your verified history, not from what you claimed. Undisclosed accidents, violations, or overstated experience put you at the entry-level rate for as long as your verified record supports it.

We will tell you what we found and what your rate is beforeit takes effect. We do not change pay for work you have already run. Report the accident. Report the violation. A driver who tells us up front is still hireable — a driver we catch is not.

I authorize Insured Transportation LLC to obtain and review, for employment purposes: my motor vehicle record from any state where I have held a license; consumer reports and investigative consumer reports, including criminal background checks; my FMCSA Drug and Alcohol Clearinghouse record, including full and limited queries; my Pre-Employment Screening Program (PSP) report; safety performance history from previous DOT-regulated employers under 49 CFR 391.23; and any other screening the company conducts as part of its hiring and qualification process. I certify that everything in this application, including my Social Security number, is true and complete, and I understand that false information may disqualify me from consideration or end my employment.
